Images & Video Consent Form

I hereby grant to For Me By Dee, hereafter referred to as the Company, its representatives and employees, a perpetual, irrevocable, worldwide, transferable, royalty-free license to use, copy, reproduce, have reproduced, modify, have modified, make, have made, sell, offer for sale, import, display, publicly perform, publicly display, distribute and prepare and have prepared derivative works of my images or videos, including those submitted by me and taken of me.

I authorize the Company, its assignees and transferees to use and publish the same in print, products, and/or electronically, including websites, publications, promotional flyers, educational materials, derivative works, or for any other similar purpose, without compensation to me.

I understand that my images and videos may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my images or videos. I also understand that this material may be used in diverse educational settings within an unrestricted geographic area.

I also understand and agree that I may or may not be identified by name and/or title in printed, Internet or broadcast information that might accompany the photographs and/or video recordings of me. I waive the right to approve the final product. I agree that all such portraits, pictures, photographs, video and audio recordings, and any reproductions thereof, and all plates, negatives, recording tape and digital files are and shall remain the property of the Company.

There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.

By signing this form I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material.

I hereby warrant that I am eighteen (18) years old or more and competent to contract in my own name or, if I am less than eighteen years old, that my parent or guardian has signed this release form below. This release is binding on me and my heirs, assigns and personal representatives.
